Mail Order Form

Each face is handmade. No two are exactly alike. All are made from polymer clay and glued on to a MDF house. Faces are not made with glasses, caps, beards or any special features to keep low prices.

House size is approximately:

7 1/2 x 8 for families of 2-9
11 x 10 1/4 for families 10-15

For bigger families, please contact me for special orders

Ordering:

STEP 1 - Specify number of faces on house and place in cart.
STEP 2 - Pick your choice of face(s) from the face sample pages.
Pick relationship for each family member (example: mom, dad, grandma etc)
- While ordering, please indicate relationship and name for face
You can order flowers and enter a saying for your plaque from inside the shopping cart

Payments are by check (w/10 day waiting period)
Please make checks payable to Linda Chavira, money order or credit cards (Visa, Master Card only)

Texas residents add 8.25% sales tax

Shipping:
$5.50 per order

All orders filled within 2-3 days

No refunds made. Make sure you double check the spelling of your names.
Any changes on faces can be made when the "proof" is emailed for approval.
Upon approval plaque is completed and mailed. No changes or corrections can be made after.

IMPORTANT - NOT INTENDED FOR USE AS A TOY. PLAQUE WILL BREAK IF DROPPED. DO NOT LET CHILDREN PLAY WITH THEM
